Violation Description: In 2018, Ofqual identified weaknesses in CIOLQ's complaints handling and application of special consideration and reasonable adjustments procedures. In January 2019, in response to concerns, CIOLQ conducted a review of its complaints handling, special consideration and seasonable adjustments policies. Between August 2019 and January 2020, CIOLQ advised Ofqual that it had made changes to its procedures, and published revised and updated policies on its website. In February 2020, Ofqual conducted a review of CIOLQ's systems for submitting a timely and accurate statement of compliance to Ofqual. The review found fundamental and significant weaknesses in CIOLQ's systems of control. CIOLQ told Ofqual that it would conduct an internal review of its procedures and processes. On 16 April 2020, CIOLQ gave an undertaking to Ofqual under General Condition B8 in which it committed to complying with an action plan of improvements and to providing regular updates to Ofqual and CIOLQ's Board.
