The Home's records are incomplete and it failed to identify the existence of a Do Not Attempt Cardiopulmonary Resuscitation decision. This may have avoided Mrs V's admission to hospital. MPS (Investments) Ltd needs to apologise, pay financial redress and improve its working practices.
