There is fault in the Care Provider's recording procedures which has caused uncertainty about what services it was providing and whether its practices were safe. It also failed to respond to Ms C's complaint properly. The Care Provider has agreed to apologise to Ms C and her father, make a payment to acknowledge the impact of the failures, reminds staff about recording contemporaneous notes, updating care plans, completing risk assessments and reviews how it deals with complaints.
